Know that I completely honor and safeguard your personal information, and that I am only requesting it to ensure safety and comfort during our meeting. 

Booking Form

Deposits are required for all new clients or for extended dates. Please be ready to take care of the 50% deposit via Cashapp or Venmo once screening is complete. In the rare event that I would need to cancel or reschedule our appointment, I will provide you with the option of having your deposit sent back to you, or holding on to it for our rescheduled meeting time. Travel and FMTY deposits can be applied to a future trip, with one week or more notice.

Include the companion's name, website & email address (no phone numbers, please)
Please include company name, position, LinkedIn, link to company website etc.
